COVID-19 Vaccination Requirements

Certain UnitedHealth Group employees are required to complete their COVID-19 vaccination series. To help protect our colleagues and the people we serve, please report your vaccination here. Click on one of the selections below and complete the requested information.

Note: If you are requesting an exemption in Alabama, Arkansas, Florida, Indiana, Kansas, Nebraska, South Carolina, Tennessee, Utah, or West Virginia, follow the specific instructions for those states listed below.

Alabama Employees: Please download this form, fill in the requested information and submit it to immunizations@uhg.com.

Arkansas Employees: If requesting a religious or medical exemption, please use the standard links above. You may also request an exemption based on COVID immunity or weekly testing. Fill in the requested information and submit it to immunizations@uhg.com.

Florida Employees: Please click on this link, follow the instructions and submit the appropriate completed form to immunizations@uhg.com.

Indiana Employees: Please download this medical exemption request form and have it completed and signed by your licensed medical provider. You may also request an exemption based on COVID immunity. Fill in the requested information and submit it to immunizations@uhg.com.

Kansas Employees: Please download this medical exemption request form or this r eligious exemption request form. Fill in the requested information and submit it to immunizations@uhg.com.

Nebraska Employees: Please download this form, fill in the requested information and submit it to immunizations@uhg.com. For a Medical Exemption request, the COVID-19 Medical Validation Form must be completed and signed by your licensed medical provider and submitted with your request.

South Carolina Employees: If requesting a religious or medical exemption, please use the standard links above. If you wish to request an exemption based on antibodies or a positive COVID test, you may download this form, fill in the requested information, and submit it to immunizations@uhg.com as part of your request.

Tennessee Employees: Please click on this link and complete the requested information.
